Osteoporosis Prevention and Treatment

If you have a family member with osteoporosis symptoms, you’ve probably noticed what I’ve seen in my own family – a gradual decline of posture, or developing a “hunchback,” with age. After spending some time with my 80-year-old mom recently, I went to give her a hug and a kiss goodbye when I was struck by how much smaller she’s become. Sure, I’ve been noticing her shrinking stature for at least a few years now, but every so often I still get surprised by how much shorter and less muscular she is throughout her upper body. Also becoming more obvious is the hunchback she’s developing, just like her mom did in her eighties.

By the time my grandmother died after breaking her hip at age 90, she had classic osteoporosis symptoms including a severe forward bend in her spine, the “dowager’s hump” as it’s called. Living for years with osteoporosis left her bones thin, weak, and fragile. This caused silent, tiny fractures in her thoracic vertebrae, the bones in the mid-section her spine. The severe rounding of her upper back was likely a sign of advanced osteoporosis, the result of compression fractures of weakened vertebrae.

What causes hunchback?

Before the common use of dual-energy x-ray absorptiometry (DEXA) to measure bone mineral density in the early 21st century, the osteoporosis symptoms of fractured vertebrae, height loss,  and forward bending of the spine, commonly known as hunchback, were the most frequent presenting complaints. The medical term for hunchback is kyphosis. Severe kyphosis is known as dowager’s hump.

Kyphosis and height loss are common osteoporosis symptoms. Decreased bone mineral density in the thoracic vertebrae causes compression fractures.  Osteoporotic compression fractures usually occur during routine daily activities such as bending of the body, coughing, lifting or some other kind of trivial trauma. Multiple compression fractures, as seen with osteoporosis, cause changes within the structure of the spine that lead to significant height loss and development of kyphosis (Dowager’s hump).

Osteoporosis symptoms from spinal fractures

Like most people with osteoporosis, my grandmother likely experienced no symptoms as her bones slowly weakened over the years. Even as her vertebrae fractured, compressed, and deformed, leading to her stooped over position, she may not have felt pain. Neither has my mother, luckily. Up to two thirds of people have no symptoms after these types of vertebral fractures. Others may suffer from quick onset of pain, which might disappear or turn into chronic, dull, aching, or intermittent back pain and stiffness. Sometimes, compression fractures of the spine cause significant back pain that may be sharp, knifelike, and disabling and which often subsides over a period of weeks to months.

As the thoracic part of the spine curves more severely, it displaces the head and neck forward, causing a compensatory increase in curve in the lower back and pushing the lower abdomen out. Even if it causes no pain, the increased curve of the low back can cause other osteoporosis symptoms such as a protuberance of the abdomen, a change in the way clothes fit, and loss of the waist. Meanwhile, the forward shifting of the head and neck can restrict the ability to look upward and therefore can affect driving and cause difficulty with lying face down.

More severe kyphosis can also cause additional osteoporosis symptoms such as loss of shoulder range of motion. This can interfere with overhead activities of daily living. With severe deformity, balance may also become impaired.  Not only is the risk for falls greater, more energy is required for standing and moving around because the spinal balance becomes disrupted and the center of gravity is displaced. Other osteoporosis symptoms resulting from more severe cases of kyphosis include shortness of breath, fatigue, and poor activity tolerance caused by the rib cage losing its ability to expand with breathing,

Too late for grandma, but not for mom or me

It may have been too late to save my grandmother from developing her severe dowager’s hump, but my mom still has decent bone mass and great mobility for a woman her age (and a breast cancer survivor, no less). Studies show she can prevent the progression of her kyphosis and the development of a dowager’s hump by staying active, strengthening her back muscles along the spine, and slowing bone density loss. Working on her posture and keeping her degenerative arthritis from progressing may also help.

Kyphosis also linked to joint disease and poor posture

There is new evidence that both poor posture and degenerative disc disease caused by osteoarthritis may also contribute to kyphosis in older people. Researchers recently began noticing that vertebral fractures don’t seem to be responsible for all cases of kyphosis. Simply having weak back muscles, poor posture, or poor health of the vertebral discs associated with degenerative joint disease can increase kyphosis. Even in cases in which vertebral fractures are osteoporosis symptoms, there is evidence that working on posture and building strength in the back muscles helps improve the kyphosis and keeps it from getting worse.

Women with kyphosis like my mom are not doomed to develop my grandmother’s dowager’s hump. And even though there is clearly a genetic component to kyphosis, neither am I. In addition to trying to eat right, taking our nutritional supplements and using other natural therapies for bone health, we’ll both be working on our posture and strengthening our back muscles with resistance training exercise. Both have been shown to be helpful for reversing kyphosis and other osteoporosis symptoms.

Of course, these are excellent methods for improving many aspects of health, not just for keeping the spine healthy and aligned. No matter what your age or bone density, there’s much you can do to prevent and treat osteoporosis symptoms such as kyphosis. Even young, athletic women need to take care to prevent osteoporosis.

Betaine HCl – One of the Best Natural Heartburn RemediesThe acid-blocking proton pump inhibitor drugs (the little purple pills), antacids, and other heartburn medications work by reducing stomach acid levels. What is betaine HCl?

Betaine HCl is a naturally occurring, acidic form of betaine, usually derived from beets. Even if you do not undergo gastric pH analysis, a trial of betaine HCl can be safely undertaken, as outlined below, for mild heartburn and indigestion relief. If you have been diagnosed with GERD or are currently taking an acid-suppressing drug, such as a proton pump inhibitor, you should not discontinue your medication on your own. Consider working with a qualified integrative physician to determine which natural heartburn remedies may be right for you.

The procedure for taking betaine HCl as one of the natural heartburn remedies, which can be found in most health food stores, is as follows:

  1. On day one, start with one capsule (usually 8-10 grains, or 500-600 mg) of betaine HCl, at the beginning of each meal (Note: This means a full meal, not a snack.).
  2. Monitor for any side effects as a warming sensation, discomfort, pain, or burning in the throat or stomach. If you experience any of these symptoms after just one capsule of betaine HCl, stop. (You likely do not have a problem with low HCl and need a different form of treatment for indigestion relief, such as melatonin or d-limonine.) If these side effect symptoms are not present, proceed to step 3.
  3. Increase your dose by one capsule (this would be two capsules or a total of 16-20 grains for day two) of hydrochloric acid (betaine HCl) at the beginning of each full meal. If you note any of the above side effects, decrease your dose to the level at which you had no side effects (the day before) and maintain this dose. If these side effect symptoms are still not present, proceed to step 4.
  4. Increase your dose as described in step 3. Each day increase the dose until you feel a warming sensation or until you reach a dose of five capsules of betaine HCl per meal. Do not take more than five (5) capsules with a meal without working in conjunction with a physician. This process will increase your stomach acid to just the right level to properly digest your food, thus working as one of the best completely natural heartburn remedies.

One precaution is that betaine HCl should not be used at the same time as aspirin, ibuprofen, corticosteroid drugs, or other anti-inflammatory medications. These medications can cause stomach bleeding and ulcers, and betaine HCl can aggravate ulcers.

Taking betaine HCl with meals can not only bring indigestion relief and be one of the most effective natural heartburn remedies; it will also improve digestion and help provide all the benefits of improved absorption and assimilation of nutrients. Fatigue, Lack of Energy

If you suffer from chronic fatigue syndrome (CFS) and knew there was compelling research showing the effectiveness of two specific nutrients in bringing relief, you would probably be interested! Such is the case, and here are the details. CFS is a complex, multifactorial illness characterized by profound, disabling fatigue lasting at least six months, which severely impairs daily functioning. Studies have uncovered a variety of hormone, immune, infectious, muscular, genetic, and nervous system abnormalities in individuals with chronic fatigue syndrome, but the exact cause (or multiple causes) remains a mystery.

Treatment within the conventional medical system focuses on controlling symptoms using cognitive behavioral therapy and exercise therapy. However, many people suffering from chronic fatigue do not improve or only partially improve with these conventional methods.  Practitioners of integrative and natural medicine take a more comprehensive and holistic approach to treatment. While not a cure-all, supplements play an important role in chronic fatigue syndrome treatment.

In addition to a high-quality, high-potency multivitamin-mineral supplement, two key supplements for chronic fatigue syndrome treatment include:

1. Adaptogen formula. Adaptogens are herbal supplements that treat symptoms of both acute and chronic fatigue and stress by increasing the body’s ability to resist stress and exerting a balancing effect on various systems of the body (immune, central nervous, cardiovascular, etc.).

A supplement that combines different herbal adaptogens such as Rhodiola rosea, Siberian ginseng (Eleutherococcus senticosus), Panax Ginseng, Schisandra chinensis, Withania somnifera (Ashwaghanda), and Astragalus membranaceus helps to re-balance and re-regulate the body’s primary stress response systems, especially the HPA axis (hypothalamic-pituitary-adrenal axis), which is dysfunctional in those with chronic fatigue syndrome.

The studies in humans examining the effects of rhodiola on fatigue are especially favorable, showing statistically significant reductions in fatigue and clinical improvements in many fatigue-related areas, such as physical fitness, concentration, mental performance, sleep patterns, motivation, mood, and general well-being.  Various dosages and preparations have been found to be effective, from daily totals of 100 mg up to 576 mg in divided doses for periods of one to ten weeks.

While very few studies have examined the effects of multiple combined adaptogens, many integrative and naturopathic practitioners prescribe formulas combining rhodiola with other adaptogens to treat fatigue and related symptoms.

2. D-Ribose.  D-ribose is needed to increase the synthesis of cellular energy in muscles. Muscle energy synthesis has been found to be lower in those with chronic fatigue syndrome. When taken as a supplement, D-ribose has been shown to enhance muscle cell energy and recovery, improve fatigue and other symptoms in individuals with fatigue and chronic fatigue syndrome in at least three studies.

In the first in a series of two uncontrolled studies in patients with fibromyalgia and/or chronic fatigue syndrome, D-ribose was administered to 41 patients at a dose of 5 grams 3 times per day for an average of 3 weeks. This led to significant improvement in energy, sleep, mental clarity, pain intensity, and well-being compared to baseline. At the end of the study, approximately 66% of patients experienced significant improvement while using D-ribose. These patients had a 45% average increase in energy and a 30% overall improvement in well-being.

A larger follow-up study, again uncontrolled, enrolled 257 patients from multiple clinics. They were given the same D-ribose protocol. Of the 203 chronic fatigue syndrome/fibromyalgia patients who completed the trial, D-ribose treatment led to a statistically and clinically significant improvement in energy (61.3% average increase).  Significant improvements were also experienced in sleep, overall well-being, pain, and mental clarity.

Another group of researchers found that 3 grams of D-ribose twice daily for three weeks in adults over 50 years of age who complained of tiredness for at least a month lessened physical fatigue as demonstrated by statistically significant improvements in tests of cardiopulmonary exercise testing. The subjects also had significant subjective improvements in mental outlook and vitality.

Both the adaptogens and D-Ribose are available as nutritional supplements that you can obtain from your local health food store or from your favorite online supplement provider. Since these two chronic fatigue syndrome treatments have a different cause of action (how they work), you can even try both approaches at the same time.

Treatment for Mold Exposure: Finding a Doctor Skilled in the Diagnosis and Treatment of Mold SicknessIn part 1 of this article, you learned that living or working in water-damaged homes or buildings can lead to a wide variety of mold exposure symptoms ranging from allergies and fatigue to more severe diseases. You were introduced to the concept that mycotoxins (the toxic byproducts secreted by molds) are the primary cause of mold-related illness, rather than an infection with the mold itself. We will now briefly discuss how this type of mold-related illness is diagnosed as well as the multiple components making up the comprehensive treatment for mold exposure. Lastly, you will learn ways to find a doctor skilled in diagnosing and treating mold sickness.

How mold-related conditions are diagnosed

The technology to detect the presence of mycotoxins in the body wasn’t available until fairly recently. Today, mycotoxins can be detected by a small number of laboratories. One of the leading laboratories for mycotoxin detection is RealTime Laboratories. They use an immunology test called an Enzyme Linked Immunosorbent Assay (ELISA) to test urine samples for Aflatoxins, Ochratoxin A, and Trichothecenes. RealTime laboratories and other labs have shown that these mycotoxins can be detected in the urine and other body tissues and fluids of people who have been exposed to toxin-producing molds in their environment, whereas people with no known exposure to molds or mycotoxins have no or very little detectable mycotoxins.

A study published in 2013 used these recently developed techniques to test urine from 112 patients diagnosed with Chronic Fatigue Syndrome. The researchers found that 93% were positive for at least one mycotoxin and almost 30% had more than one mycotoxin present. Ochratoxin A was the most prevalent mycotoxin detected (83%) with trichothecenes as the next most common (44%). Over 90% of those positive for mycotoxins reported current and/or past exposure to a water damaged environment.

Mold exposure treatment

While antifungal drugs or natural antifungal agents can be used to treat direct fungal infections (when the fungus itself is growing within the body), treatment for mold exposure is more difficult and does not respond to antifungals since the mycotoxins are typically present without the presence of the fungus itself. The treatment of illnesses caused by mycotoxin exposure begins, first and foremost, with avoidance of further exposure to water-damaged environments and items contaminated by those environments.

Many healthcare practitioners with knowledge and experience in working with mold-related illnesses also recommend decreasing exposure to other toxic chemicals including pesticides, heavy metals, and volatile organic compounds and fragrances in order to reduce total toxin exposure and improve the body’s detoxification abilities.  Another mainstay of treatment is the use of what are known as sequestering agents. These are non-absorbable materials such as activated charcoal, clay, chlorophyll, and the drug cholestyramine, which are capable of binding toxins in the gastrointestinal tract, thus reducing re-circulation and ultimately lowering the total toxic load.

The use of antioxidants, especially glutathione, and the treatment of nutritional deficiencies, especially vitamin D, magnesium, zinc, coenzyme Q10, and B vitamins, is also reported to be helpful. Probiotics and various dietary therapies, especially the identification and avoidance of foods to which one is intolerant, are also commonly used along with the treatments mentioned above.

Finding a doctor who specializes in mold exposure treatment

Do not confuse this organization with the American College of Occupational and Environmental Medicine which officially denies the existence of adverse health effects related to the inhalation of mycotoxins.

Osteoporosis Prevention and Treatment

Osteoporosis is an insidious illness that sneaks up on you. Studies suggest that about 50% of women over the age of 50 will suffer a fracture related to bone loss. And each year, approximately 80,000 men have a hip fracture. That’s why it’s so critical to undergo bone density testing. Even if you are otherwise “healthy”, getting a baseline bone density test can predict even the slightest beginnings of bone loss in your future.

There are a variety of bone density tests available, but the “gold standard” test for diagnosing osteoporosis is the DEXA scan (dual energy X-ray absorptiometry), which measures bone density in the spine, hip or wrist. If you do have low bone density, these are the most common locations for fractures.

Does the DEXA scan hurt?

A DEXA scan is painless and only takes about 15 minutes to perform. During the test, you lie on your back on a table and a scanner passes over your body taking radiographic images.

Who should get a DEXA scan?

DEXA screening is recommended for all men over the age of 70 and all women over the age of 65. Bone density should also be measured in women between the ages of 50 and 65 who are considered high risk for osteoporosis. In addition to obtaining an initial DEXA scan, repeating the scan every 2 to 5 years is recommended, depending on your risk factors.

What about radiation exposure?

The radiation emitted from a DEXA scan is about one-tenth the radiation emitted from a chest x-ray. Still, there are other bone density tests which emit less or no radiation at all: QCT scan, NTx urine test, or vitamin D test. Interpreting your bone density scores

DEXA bone density scores are measured as “T-scores”, which is a comparison of a person’s bone density with that of a healthy 30-year-old of the same sex. The lower bone density scores, the lower your total bone density.

Bone Density Chart: Understand Your Bone Density Scores A T-score of -1.0 to -2.5 signifies osteopenia, meaning below-normal bone density without full-blown osteoporosis. This stage of bone loss is the precursor to osteoporosis.

To better understand the current health of your bones, you should multiply your T-score by 10% (as shown in the bone density chart below). This will give you a rough estimate of how much bone density has already been lost.

Focusing too closely on the DEXA bone density scores can be a mistake!

Many doctors admit that the DEXA T-score is not a perfect predictor for bone health or fracture risk. That’s why it’s important to consider taking the other tests described in our Guide. Furthermore, your risk factors are just as important as your T-score and may lead to better predictions of bone disease. Vitamin B3

Niacin, or Vitamin B3, actually works by raising your HDL (“good”) cholesterol levels. According to the Mayo Clinic, “Taking niacin — either by itself or along with other cholesterol-lowering medication — may help control your total cholesterol level.”[2] Niacin works in the liver by affecting the production of blood fats. It is so well researched and the evidence for using it to decrease high cholesterol is so strong that it is recommended by the National Cholesterol Education Program. Fatigue is a common symptom in people with medical conditions like cancer and diabetes, mood disorders like depression and anxiety, and even in people who are otherwise “healthy”.  Yet despite how prevalent it is, medical researchers still do not completely understand what happens in the body to cause fatigue. They hypothesize that fatigue typically results from a complex set of interacting factors, rather than a single cause. A number of these factors have now been identified and studied.  Here we take a look at five of the most common fatigue causes.

The top five fatigue causes:

  1. Stress.  Chronic stress often leads to dysregulation of the body’s stress systems and is a common underlying cause of chronic fatigue.[1] The popular term “Adrenal Fatigue”, with its focus on the adrenal glands and cortisol production, is often used to describe this unhealthy reaction to chronic stress.  However, “Adrenal Fatigue” does not take into account the complex system of interconnected events involved in the stress response. The brain, nervous system, endocrine (hormonal system), and immune system are all key components, in addition to the adrenal glands. In particular, the hypothalamic-pituitary-adrenal (HPA) axis and the sympathetic nervous system (SNS) are the primary regulatory centers.  Fatigue is a common consequence of excessive, prolonged or inadequate regulation of these stress response systems.[2] Find tips on beating stress here.
  2. Poor nutrition/digestion.  Nutrition and digestive issues such as nutritional deficiencies, food allergies, blood sugar dysregulation, and excessive permeability in the lining of the intestines can all cause fatigue. Those suffering from fatigue are commonly eating a poor quality diet (low in fruits and vegetables; deficient in vitamins, minerals, and phytonutrients; excessive in high-glycemic, high-fat, high-inflammatory foods).   3. Hormone imbalances.  Fatigue may result from low or imbalanced levels of common hormones such as thyroid, testosterone, estrogen, and progesterone.  Low thyroid hormone levels resulting from autoimmune thyroid disease is one of the most common hormone-related disorders, and fatigue is the main symptom of hypothyroidism. In women, menopausal symptoms such as night sweats are notorious for causing fatigue [4], while in men testosterone deficiency as a cause of fatigue is now being recognized more than ever before.[5]
  4. Anxiety and depression.  Mood disorders such as depression and anxiety, the most common mental health disorders in the US, either cause or are strongly associated with fatigue and daytime sleepiness.  The majority of people with depression suffer from fatigue. In fact, fatigue is one of the most common symptoms of depression that patients complain to their healthcare providers about.  In one recent study, more than 90 percent of patients with major depression had severe fatigue despite the fact that more than 80 percent of these patients were already taking antidepressant medications.[6] Depression-related fatigue is often associated with impaired concentration, irritability, and reduced productivity.
  5. Sedentary lifestyle and deconditioning.  Being highly sedentary increases the risk of chronic fatigue. Not getting enough exercise and physical activity causes deconditioning of the body’s musculoskeletal and cardiovascular systems and depresses mood, all of which lead to fatigue. Studies show that the more regular physical activity you get, the more likely you are to feel energetic. Exercise has been shown to help reverse fatigue associated with all kinds of medical conditions, from cancer to autoimmune diseases to depression.[7] Find more reasons to exercise here.

Maximum serotonin response occurs when you take supplemental 5-HTP on an empty stomach between 3:00 and 4:00 p.m. and again at bedtime. Start off with the lowest dose – 50 mg – take at each of those times with 4 ounces of juice.

After an initial three days of 5-HTP at 50 mg twice daily, increase the dosage to 100 mg at each of the two times and stay at that level. If this doesn’t do the job within four to six weeks, double the before-bed dose to 200 mg. Also, make sure to take a good B-complex vitamin during the day (see below). Though having too little 5-HTP is not good for you, taking too much 5-HTP can lead to harmful consequences as well.   • B12: Wild salmon, halibut and shrimp, grass-fed beef (especially beef liver), lamb, venison, eggs and yogurt.
  • B6 (Pyridoxine): Chicken, turkey, tuna, shrimp, beef liver, cheese, beans, spinach, carrots, brown rice and sunflower seeds.
  • B9 (Folic Acid): Spinach and other green, leafy vegetables, asparagus, turnips, beets, Brussels sprouts, avocadoes, beans, beef liver, salmon, and orange juice.

How to Take B Vitamins as Natural Remedies for Panic Attacks and Anxiety

B12: B12 can be given as an injection (called cyanocobalamin injections) or as a nasal spray by an integrative physician. Doctors typically administer injections in doses of 1000 micrograms. Initially, an injection cycle of eight to 10 administrations over the first two to three months is recommended, followed by a decrease in the frequency once you have achieved a therapeutic effect.

If you object to injections, oral supplementation with vitamin B12 is safe, efficient, and inexpensive. Most multivitamin pills contain 100 to 200 micrograms of the cyanocobalamin form of B12. This must be converted to methylcobalamin or adenosylcobalamin before it can be used by the body.

The actual absorption of B12 is a problem with supplements; therefore, to increase absorption, B12 should be taken along with the other B vitamins. But, another effective approach is to dissolve a sublingual tablet of methylcobalamin (1000 micrograms) under the tongue once or twice every day. This form of sublingual B12 is readily available as an inexpensive supplement.

Since B12 is so important yet hard for the body to absorb in the digestive track, many alert consumers take a sublingual B12 along with their regular B complex supplement.

B6: Vitamin B6 is also sold under the names pyridoxal, pyridoxamine, pyridoxine hydrochloride, and pyridoxal-5-phosphate and can be found in oral supplements and liquid drops. Adults ages 19 to 50 years require 1.3 mg of B6 daily. Women 51 years and older require 1.5 mg daily. Pregnant and breastfeeding women require 1.9 mg daily.

B9: Vitamin B9 (folic acid) works with vitamins B6 and B12 and other nutrients to control blood levels of the amino acid homocysteine. It also works best when it is taken along with the other B vitamins; thus, taking a B vitamin complex supplement is best. Adults require 400 micrograms of folic acid per day. Pregnant women require 600 micrograms per day and breastfeeding women require 500 micrograms per day.

Natural Remedies for Panic Attacks and Anxiety #3 – Iron

Like the B vitamins, iron plays an important role as a cofactor for the synthesis of serotonin. Having low iron levels can lead to anemia and has been associated with fatigue, weakness, depression, anxiety and panic attacks.[3] According to the World Health Organization, iron deficiency is the number one nutritional disorder in the world.

Women ages 19 to 50 require 18 mg of iron daily as iron deficiency is often caused by menstruation. Post-menopausal women and men require 8 mg of iron daily.

Iron is a tricky mineral. While having too little iron is associated with panic attacks and can lead to anemia, having too much iron in the body can lead to a condition known as hemochromatosis, which can cause diabetes, liver damage, and discoloration of the skin. Unlike other nutrients, excess iron cannot be excreted by the human body. Therefore, you should always ask your doctor to measure your iron levels though lab testing and you should never take iron supplements on your own. Also, iron should never be given to infants, children, pregnant or breastfeeding women except under the direction of a doctor.

Calcium is the link between vertigo and osteoporosis

While vertigo can be triggered by a number of underlying issues (inflammation or fluctuating pressure in the inner ear, Meniere’s disease, etc.) one major cause is loose calcium carbonate crystals moving through the sensing tubes of the inner ear. The inner ear governs sense of balance.

Both vertigo and osteoporosis occur more often in post-menopausal women because the drop in estrogen that accompanies menopause causes bones to release more calcium into the blood. And, the increase in “free” calcium also prevents the body from clearing the crystals out of the inner ear as it normally would.

“Women most often have their first case of vertigo in their 50s, when they are also having a drop in bone mass due to loss of estrogen,” says study author Ji Soo Kim, MD, PhD, of Seoul National University College of Medicine in Korea.

Don’t forget that calcium is not the only nutrient necessary to promote healthy bones, and it may not even be the most important one. In fact, calcium by itself does little good because it is not as readily absorbed by the body without taking magnesium and vitamin D.  Furthermore, eating or supplementing too much calcium in the absence of other nutrients can actually lead to osteoporosis and cause dizziness as well as other serious problems such as kidney stones, plaque in your arteries or painful calcium deposits manifested as gout or arthritis. Urine NTX and Serum CTX Test—Measuring Bone Breakdown Compliments DEXA Scan for OsteoporosisBone resorption testing is becoming known as an accurate and inexpensive way to measure the rate of bone loss. Many integrative physicians now see these labs, such as the urine NTX or the serum CTX test, as invaluable in evaluating osteoporosis, since they can identify excessive bone loss before too much damage has occurred. Also, they allow for more regular, frequent assessment of bone health status than do bone density scans. This means that treatments can be started to slow bone loss before it would even have been identified by a DEXA scan. And treatment results can be monitored more frequently and thus adjusted for maximum effectiveness.

There are multiple bone resorption tests to choose from: at least four different markers of bone resorption can be measured in the blood, and at least seven can be measured in urine. So far, it’s not yet clear exactly which one, if any, is best to use. In part 1, you learned about bone remodeling in general and about the Pyrilinks-D urine test for the bone resorption marker known as deoxypyridinoline (DPD). Two other popular, commonly used bone resorption tests are the urine NTX and the serum CTX test. These two tests along with Pyrilinks-D have emerged as the three most useful.

What are “CTX” test and urine “NTX”?

CTX and NTX are acronyms for carboxy-terminal (CTX) and amino-terminal (NTX) telopeptides of type 1 collagen. Don’t let this mouthful scare you off from learning about these important tests. These short proteins (peptides) make up certain regions of bones’ collagen. Collagen, specifically type 1 collagen, is the substance that makes up the majority of the non-mineral tissue of bone.

Collagen forms the matrix upon which the mineral portion of bone accumulates. This collagen is strengthened by cross-linked proteins such as DPD, which is measured by the Pyrlinks-D test (see part 1). CTX and NTX are found in the region of type 1 collagen where the crosslinks such as DPD attach. They are produced during bone resorption and can be measured in blood or urine. Currently CTX is most commonly measured in blood (serum) while NTX is most often measured in urine.

What do high urine NTX or serum CTX test levels indicate?

High serum CTX test levels have been found to correlate with low bone mineral density and to predict fracture risk. Serum CTX test levels are higher in people with osteoporosis compared to those without the disease.[1] In a recent published Swedish study, 1,040 women aged 75 years were randomly selected and followed for ten years. High serum CTX test levels were associated with increased fracture risk for up to a decade.[2] The urine NTX test is also commonly used and recommended as a good bone resorption marker. A study of 2,305 women found that higher urine NTX levels in the urine before menopause and across menopause is associated with a higher risk of fracture.[3]

What are the normal ranges for the urine NTX and the serum CTX test?

The serum CTX test and urine NTX test are offered through LabCorp, Quest Diagnostics, and other major national labs. As with the other bone resorption tests, the various measurement techniques for CTX and NTX lead to reference ranges that may vary from lab to lab.

  • For urine NTX measured through LabCorp, for example, a baseline urine NTX value greater than 38 indicates significantly increased risk for decreased bone mineral density after one year.[4] As the baseline NTX goes up from 38, the probability of a decrease in bone mineral density increases significantly. For women being treated for osteoporosis, the probability that treatment is effective after three months is significantly increased if the NTX falls below 38 or drops by 30% from its baseline.[4] The retail cost for the urine NTX (N-telopeptide) test is between about $60 and $120.
  • For a serum CTX test through Quest Diagnostics, the normal range for women aged 18-49 ranges from 40-650 pg/ml. For patients being treated for osteoporosis, a serum CTX decrease of at least 25% from baseline 3 to 6 months after the start of therapy indicates the therapy is working. Life Extension’s retail price for the serum CTX test, which is run through LabCorp is $332.

Which is better, the serum CTX test or urine NTX?

Different research studies have used different markers, and experts vary regarding which markers they recommend. For these reasons and others, doctors have been reluctant to begin regularly using bone resorption tests in clinical practice. The serum CTX test, also called C-telopeptide, is the test that some experts are currently recommending becomes the primary bone resorption test used worldwide so that international reference ranges can be set, research can be more standardized, and the test can be more useful to doctors and patients.[5]

The main reason the serum CTX test is being recommended over the other bone resorption markers is that not all markers respond by the same amount for a given degree of bone resorption; for instance serum CTX tends to change more than urine NTX.[5] Nevertheless, both are useful and currently in use along with Pyrilinks-D. Your doctor may have a preference based on his or her personal experience with one test over another.

How to best take advantage of bone resorption testing using Pyrilinks-D, urine NTX, or the serum CTX test

Like most lab tests this day in age, bone resorption tests can be ordered without a doctor’s recommendation or guidance through various online companies that offer direct-to-consumer access to lab tests. DirectLabs or Life Extension are examples. However, given the complexity of bone resorption testing, it is recommended that you work with a healthcare practitioner experienced using and interpreting Pyrilinks-D, urine NTX, or the serum CTX test.

Osteoporosis is a silent disease causing major loss of mobility and even death. Fracture risk depends on much more than just bone mineral density as measured by a DEXA scan. If you are already at risk for fractures, or if you want to know if you are at risk, testing for bone resorption is emerging as an integral way to help assess and treat poor bone health. No matter what your current state of bone health, if you want to start tracking your bone loss rate, it is important to establish a baseline.

The best way to take advantage of a urine NTX or serum CTX test is to find out your initial level of bone resorption and then plan to re-test 3 to 6 months later using the same test at the same laboratory. This can help you determine whether you are currently losing too much bone. It may provide an early warning before a bone density scan reveals a problem. Or you can use the testing  to determine how well your current bone building plan is working. If your bone resorption levels have not improved on the follow up test, those results will serve as a great motivator for you to be more vigilant with your weight bearing exercise and other natural treatments. In terms of the common cold, it is now well-accepted that acute respiratory infections are frequently experienced after air travel. Studies have found a high prevalence and wide array of respiratory viruses in people who have recently traveled.[2] Up to 20% of passengers may develop respiratory infections within 1 week after air travel and that flight attendants have significantly more respiratory infections than those who do not frequently fly.

Why does flying on a plane increase your susceptibility to getting sick?

  • Spread of exhaled infected droplets. Airline cabins have lots of people in a crowded space who are there for long “exposure times” so the risk of infection via airborne transmission could be high if one or more passengers are infected with an airborne infectious disease. The droplets exhaled by an infected passenger may contain infectious agents such influenza. The droplets have been shown to spread through the air from a single cough, a single breath, or a few seconds of speaking. Exposure to the row in front and the row behind occurs within 30 seconds and extends to 7 rows within minutes.[3]
  • Low air pressure and oxygen compromise immune function. The conditions of low air pressure and low oxygen have been shown to impair certain immune system functions for a few days after long flights.[4] To test how typical airplane air pressure and oxygen levels affect the immune system, researchers studied the effects of a simulated 10 hour flight at an average cruising altitude of 8000 feet; and they measured markers of immune system function in 52 healthy volunteers. They looked at blood samples before and on days 1, 4, and 7 after the flight. The researchers observed a number of immune system changes in the immediate days following the simulated flight. For instance, the subjects’ lymphocyte responses were decreased. Lymphocytes include T cells, B cells, and natural killer cells. Lymphocytes are the primary type of white blood cells made by the immune system to combat viruses, bacteria, and other pathogens. These immune changes, they concluded, “may contribute to an increased susceptibility to respiratory infections commonly seen after long-haul flights.”[4]

Venous thrombosis is also a health concern related to air travel

The lower oxygen levels and prolonged sitting on long flights doesn’t just impair your immune system’s ability to fight infections, it also puts you at risk for deep vein thrombosis and pulmonary embolism. Together, these two conditions make up the third most common cardiovascular disease after heart attack and stroke. For persons who contract the conditions, they carry a high risk of death and permanent complications. Deep vein thrombosis is a condition in which a blood clot (thrombus) forms in one or more of the deep veins in your body, usually in your legs. Deep vein thrombosis can cause leg pain, but often occurs without any symptoms. Pulmonary embolism is a complication of deep vein thrombosis in which one or more arteries in your lungs becomes blocked by a blood clot that has traveled to your lungs from your leg.

While these conditions are of most concern for people with chronic respiratory diseases, doctors who specialize in travel medicine and lung diseases have been trying to spread the word for years that all travelers need to take precautions to avoid venous thromboembolism. This involves avoiding excess alcohol and caffeine-containing drinks, remaining mobile and exercising during the flight. For air travelers with a history of venous thromboembolism, obesity, heart disease, significant medical illness in previous six weeks, immobility, pregnancy or estrogen therapy (hormone replacement  therapy or combined oral contraceptive pill), experts advise that in addition to the above advice, travelers wear compression stockings, avoid sedatives, and avoid sleeping for prolonged periods in abnormal positions. They also recommend a preventive dose of blood thinning medication for those at high risk of clots, such as those who have recently had a stroke, major surgery or trauma.

How to not get sick while flying

To reduce your risk of getting a respiratory infection while on a flight, here are three specific actions you should take:

  1. Irrigate your nose (or your children’s) with a saline solution – both before and after the flight – by using a dropper or small squirt bottle. Nasal irrigation with a saline solution helps clean out at least some of the allergen, fungal and viral pathogens from the nose and thus can help to reduce the frequency of infections. Keeping the nasal cavity moist in the dry atmosphere of an airplane cabin is also most beneficial.
  2. Drink lots of water throughout the flight.
  3. Take an herbal medicine with immune boosting properties such as Echinacea for a week or two before the flight. Australian researchers conducted a randomized, double-blind placebo-controlled trial to study whether a standardized Echinacea supplement is effective in the prevention of respiratory and other symptoms associated with long flights.[5] They gave 175 adults travelling for 15-25 hours Echinacea (root extract, standardized to 4.4 mg alkylamides) or placebo tablets starting 14 days before travel. The Echinacea group had lower respiratory symptom scores compared to placebo during travel. 4 Dopamine Supplements for Improving Mood and MotivationWhen we think about depression, lack of motivation, or difficulty focusing and concentrating, the well-known brain chemical (neurotransmitter) serotonin often comes to mind. While it’s true that low serotonin is a problem for many people with depression and other mental health issues, researchers have known for years that other neurotransmitters are also involved. A study published in JAMA Psychiatry in the year 2000 reported that people with clinical depression also have significantly lower brain levels of the neurotransmitter dopamine.[1]

Since then, we’ve learned a lot about dopamine’s role in mood and mental health and about what can be done to restore low dopamine levels. Loss of pleasure,  loss of motivation, and not having enough focus or concentration to get things done  can all be dopamine deficiency symptoms, as can the characteristic “slowness” of many people with depression. While most pharmaceutical drugs designed to increase dopamine are associated with significant and serious adverse effects, scientists have discovered and clinically tested a number of natural dopamine boosters that can safely increase dopamine levels within the brain and are generally without side effects. Natural and integrative physicians have been successfully using these ingredients to help patients with depression, anxiety, low motivation, and other symptoms of low dopamine.

What is dopamine?

Dopamine normally gets triggered when you approach and expect a reward. With the release of dopamine in the brain comes a good feeling and a surge of energy so you can reach your reward. Dopamine motivates you to seek, alerts your attention to things that meet your needs, and motivates you to persist in your pursuit of those things that meet your needs. Your brain rewards you with dopamine each time you take steps towards a new goal. Without enough dopamine, your motivation goes caput and you’re unable to experience pleasure from activities usually found enjoyable, e.g. exercise, hobbies, music, sexual activities or social interactions. In other words, dopamine deficiency causes a bad case of the “blahs.”

The four supplements presented here—L-Tyrosine, Rhodiola, Mucuna, and L-theanine—have each been found in studies to increase dopamine and/or help balance dopamine function in the brain. They can be used as natural dopamine boosters to improve mood and motivation and to treat dopamine deficiency symptoms like depression, fatigue, lack of interest in life, poor memory, and impulsive behaviors.

4 Key Dopamine Supplements:

L-Tyrosine. The conditionally essential amino acid tyrosine is a precursor of catecholamine neurotransmitters, including dopamine. It can be taken through the diet (especially from meat, eggs, and fish) or synthesized in the body. Tyrosine forms DOPA, which is then converted to dopamine, and this, in turn, forms norepinephrine, another neurotransmitter related to mood. By supporting production of neurotransmitters like dopamine, L-tyrosine supplements can enhance mood, sleep, emotional well-being, and cognitive/mental function, especially under situations involving environmental and emotional stress or when dopamine levels require additional support (some people are genetically programmed to make too little dopamine).[2-4] Start by taking one 500 mg capsule of L-tyrosine. If you feel no benefits within 30 minutes, take a second capsule, and a third in another thirty minutes if you still feel nothing. Continue by taking one to three 500 mg capsules 2-3 times a day, early morning, midmorning, and midafternoon. Decrease the dose if you feel agitated or your blood pressure increases.

Mucuna. Mucuna pruriens, commonly known as velvet bean, naturally contains up to 5% L-Dopa (levodopa). L-DOPA is the same biochemical that is made in humans from the amino acid L-tyrosine and is then synthesized into dopamine. When taken as a supplement, the L-DOPA from Mucuna can cross the blood-brain barrier to elevate brain dopamine levels. Powdered mucuna seeds have long been used in Indian traditional medicine as support in the treatment of various illnesses, including Parkinson’s. Recently, studies utilizing Mucuna supplements have shown promising results not just for Parkinson’s but for other conditions related to dopamine deficiency, including depression and psychological stress.[5-7] Mucuna extract has been shown to increase not only dopamine concentrations, but also other neurotransmitters that affect mood, such as serotonin and norepinephrine.[8] Look for an extract of Mucuna pruriens standardized to contain 15% L-DOPA. Take 300 mg twice a day.

L-theanine. L-theanine is an amino acid uniquely found in green tea that creates an alert state of relaxation without drowsiness. L-theanine is known to be able to cross the blood-brain barrier and increase dopamine levels in the brain. Animal studies show that L-theanine also increases brain serotonin and GABA. It has anti-depressant and anti-anxiety effects, reduces mental and physical stress, and leads to improvements in learning and memory in humans and animals.[9,10] Even just a single, small dose of L-theanine (100 mg) significantly improves the ability to pay attention and maintain focus compared to placebo.[11] Take 200 mg of L-theanine 2-3 times daily.

Rhodiola. Rhodiola rosea, or ‘golden root’, is a popular plant in traditional medicine in Eastern Europe and Asia, with a reputation for improving depression, enhancing work performance, eliminating fatigue and treating symptoms resulting from intense physical and psychological stress. Rhodiola exerts its benefits via multiple effects on the central nervous system, including enhancing the stability of dopamine and supporting its reuptake. This leads to notable decreases in depression, anxiety, and fatigue, as well as an increased ability to handle stress.[12] In human studies, rhodiola has been shown to significantly reduce depression, anxiety, and stress-related fatigue compared to placebo.[13-15] Look for a rhodiola extract derived from Rhodiola rosea root and standardized to contain 3% total rosavins and a minimum 1% salidrosides. Take 170 mg twice a day.

Multivitamin. Certain minerals and B-vitamins, especially zinc, vitamin B6, and folate, are necessary for dopamine synthesis and neurotransmission. These nutrients are often depleted in individuals due to medications, inadequate diets, excessive stress, and toxic environmental exposures, compromising the ability to properly synthesize neurotransmitters like dopamine. A high-potency, high-quality multivitamin/mineral supplement can help replenish these co-factors, enhancing neurotransmitter function and playing a complementary role in supporting emotional wellness

Potential side effects, precautions, and drug interactions

Too much dopamine is dangerous and needs to be avoided. Do not take more than one dopamine supplement at a time without first consulting with a healthcare practitioner, preferable one trained in integrative or natural medicine. Similarly, do not use these supplements if you are taking methyldopa, antidepressants, or antipsychotic drugs without first consulting with a physician. Tyrosine and Mucuna pruriens may also interact with some nutritional supplements, including St. John’s Wort, 5-HTP, Tryptophan, and SAMe. Therefore, you should also consult your healthcare practitioner before combining these supplements. Do not take these supplements if you are a pregnant or lactating woman.

Additional ways to increase dopamine and improve your mood

In addition to taking dopamine supplements, there are also other ways to naturally increase dopamine. For instance, do you know which foods are natural dopamine boosters and which foods can deplete dopamine?  Working towards a goal can also increase dopamine. By repeating small steps to reach a goal, you can re-wire the dopamine pathways in your brain, ultimately teaching your brain to give you a dopamine surge every time you take that small step. You can also increase dopamine by developing an active, regular, stress reduction practice.[16]
